At the end of the survey, the form gives the user the option of entering his name and email. How do I disable that? I don't want the user to be able to give his name and email.

Just like you, I want to have one page survey.Just questions, answers and "Submit Survey" button.
My wordpress theme is a theme without page name in url.I don't want the users to be able to give his name and email, and I don't need a thanksgiving text.

http://microlab-deva.ro/ and the survey page is where it writes "CHESTIONAR" in Romanian.I use ShapeShifter theme.
