Hi, Vinny.
It takes 3 clicks of next to advance from last survey question to email input form. I am afraid some people are responding but giving up without submitting survey. Can you please let me know if this is a bug and whether there is any way to fix it?

It works correctly for me - can you post the URL of your survey here so that I can take a look at it?
