Please help

I uploaded the files to wp-content>plugins then activated the Surveys plugin.
Then went to Tools and clicked on Manage Surveys --- The page cannot be found

the adress bar read: http://demforum.com/blog/wp-admin/surveys/survey.php

?? Since it seemed to want to look in wp-admin, I uploaded the survey folder there.
Now when I go Tools>Manage Surveys I simply get a blank page.

I am using Wordpress version 2.8.6

Actually, the link should be something like...
http://demforum.com/blog/wp-admin/tools.php?page=surveys/survey.php

And you don't have to have to upload it to wp-admin/ - just uploading toh the plugin folder is enough.

Perhaps this issue is caused by other plugins - which all plugins do you have installed?
